Project Connections provides health related services for children 0-5, pregnant mothers and their families living in Brea, La Habra or Fullerton
- Health Insurance Application Assistance: Medi-Cal, Healthy Families and others
Are you interested in applying for Medi-Cal or Healthy Families? Let us help. To find out if you qualify, you will need:
o Proof of income and deduction (pay check stub, child care expenses).
o Proof of residency (birth certificate).
o Proof of pregnancy (if applicable).
- Developmental Screenings
- Health education classes
- Immunization screenings
- In-home health and safety education for parents with newborns
- Medical home visits with a Public Health Nurse or Promotora/Health Advocate
- Referrals to community agencies and support services
